For youth navigating independence without that foundation, having a consistent, trusted person in their corner can make all the difference. This program meets young people where they are and helps them build the confidence, skills, and connections they need to move forward on their own terms.

Building the Skills to Stand on Your Own

The Youth Transitioning to Adulthood program supports youth ages 13–18 who are living independently or facing social, emotional, or behavioural challenges. Youth Outreach Workers help participants build life skills, practice self-care, secure stable housing and income, and navigate social services, mental health care, and community resources. The program begins with a referral and individualized service plan tailored to each young person’s goals and needs. Funded by the Province of British Columbia, it provides structured support to help youth gain confidence and move toward independence.

Eligibility & Referral

Ministry of Child and Family Development (MCFD) Referral, and/or Youth aged 13-18 and experiencing issues related to youth independence and wellbeing.
